We are a CCSP/SOURCE management referral agency for those individuals who are in need of placement in a personal care home. At the moment, we offer care partnership opportunities with Family Model Personal Care Homes. The personal care home must be licensed and registered by the State of Georgia Department of Community Health, Office of Health Care Facilities Regulation. In addition, the personal care home must be licensed to care for 2-6 residents, cannot be under contract with another agency, and cannot have any deficiencies.
